simply mail a Request for Reimbursement with a copy of the Receipt email (including header, [to: from:]),  Union Card, Photo ID (Drivers License, Passport, or other Federally accepted Photo ID)  to:

Mechanic Licensing 50% Filing Fee Reimbursement For NY & NYC license

Attention: The deadline for Mechanic Licensing set by NY Department of Labor is January 1, 2022.

  1. Take 1 Picture of Union Card, Photo ID (Drivers License, Passport, or other Federally accepted Photo ID) and license fee receipt (see red note above).
  2. Fill out Request form, on the form upload the Picture in the field provided(See Below Blue Button for form). Only Request what you are Eligible for, see note above. (A Gmail Account required for Uploading Documents to the form. When filling out the form, type in the email you check regularly, like a work email address.)
  3. Once your Request has been processed, you will receive an Email to the Email address Provided With its status and If approved, A reimbursement Check will be issued and mailed to the provided address within 8 weeks.

Request form is only for Local #3 IBEW Elevator Division members. All others will be Rejected.
